To get your MMJ license, better known as a “Red Card”, you fill out an application form, name a provider, obtain a recommendation form from a physician, pay the necessary fees (doctor visit and state application) and submit the paperwork to the Colorado Department of Public Health & Environment.
You will have to renew your card annually by submitting a renewal form, physician’s recommendation form, copy of your ID and the current fee. (This page was last updated 10/4/2011)
- Fill Out Your Application – The CO state MMJ website can be found here and has all of the up-to-date information as well as the most current forms. Most doctor’s offices have these forms available, but’s probably not a bad idea to take a look at the form and make sure you have all the information you need and are using to correct version.
- Name a Provider – On the application you will be required to name a “Provider”. A “Provider” is allowed to grow a certain amount of inventory for each person that names them as their “Provider”. The “Provider” maybe a person, yourself or most likely your local dispensary. Dispensary’s usually have special pricing for MMJ card holders that name them as their “Provider”. THC’s member pricing is listed here. You can change your designated “Provider” by filling out the appropriate form, having it notarized and sending it to the state. Many dispensaries have a notary on site and can help you with making the change.
- Physician Recommendation – There are a variety of doctor’s offices established for the sole purpose of evaluating your particular case and making an MMJ recommendation. (See the next section to review a list of qualifying conditions.) Some doctors require medical records others do not. Most visits will cost between $ 55 – $ 100. If you are renewing your card some doctors offer the visit at a reduced fee. The physician’s office will give you a completed recommendation form. The application paperwork must be notarized and most doctor’s offices have a notary available, usually for an extra fee of about $5. - Pay the Fees – The annual fee is $ 90.
- Submit Your Paperwork – Once you have your application and physician recommendation forms completed and notarized you submit them with the fee and a copy of your ID, usually a Drivers License. You can leave your paperwork at the drop box or send it by certified mail. There really is no reason not to send the paperwork by certified mail, you’ll want the receipt.
- Doing It Right – All of the forms have instructions but in short: follow the directions, use blue ink, don’t cross out information, start a new form if you need to, process everything at the same time, and be sure to keep copies of everything.
- Now What – MMJ Cards should arrive within 2 – 3 weeks. Some dispensaries, particularly if you’ve named them as your “Provider”, will accept copies of your completed paperwork (including the certified mail receipt) as a temporary card. They will verify with the state that your application hasn’t been denied each time you visit. The “35 Day Rule” states that if after 35 days you still haven’t received your “Red Card” then the application is considered approved and your copies can be used as a temporary card.

Inside the first set of glass doors will be a Drop Box for MMR applications. Doors open: Monday-Friday, 7:00 a.m. to 6:00 p.m. Your application must be in a sealed envelope. You will not receive a receipt. If you wish to have a receipt, please mail in your application by certified mail.

Patients who have had a diagnosis of a debilitating medical condition in the past but do not have an active disease and are not undergoing treatment for such condition are not considered to be suffering from a debilitating medical condition for which the medical use of marijuana is authorized.
